Root canal treatment has one clear goal: to preserve an inflamed tooth for as long as possible and avoid extraction. The root canal system of a tooth is often considerably more complex than it appears on a standard X-ray – canals can be very narrow, strongly curved, calcified, or hidden in hard-to-reach locations.
A typical example is the upper first molar. It usually has three roots – but in a large proportion of patients there is an additional, fourth canal, known as the MB2 canal. Reported prevalence varies by population and examination method: systematic reviews describe rates of around 60–75%, and some clinical studies on upper first molars report figures as high as 90%. If such a canal goes undetected, bacteria can remain inside the tooth – a recognised reason why a previously root-canal-treated tooth may become painful again and require retreatment.
